Service Description

This is a three month program and includes a 90 minute consult, x3 one hour follow up visits (one per month) as well health coaching with the Nurse Practitioner. The Provider will review all your medical history from birth until present. Recommendations will be provided on how to move forward with diagnostic work-up which will likely include speciality testing. The client will have direct communication with the Nurse Practitioner via text messaging, voice calls, or video chat for more on demand needs.
